Dear players,

The "Jockey Club Youth Football Development Programme - Summer Scheme 2018" is funded by The Hong Kong Jockey Club Charities Trust, organized by the Hong Kong Football Association, and evaluated by the Department of Sports Science & Physical Education of The Chinese University of Hong Kong (SSPE/CUHK). We now invite you to take part in the participant evaluation to help us gain valuable insight into the “Jockey Club Youth Football Development Programme”.

Requested by The Hong Kong Jockey Club Charities Trust (funding organization), all participants MUST fill in the questionnaires. Please answer the questions truthfully according to your own knowledge. There are no correct or incorrect answers, choose the answer that best reflect your status.

Every participant will be required to fill out questionnaires at 2 different time points: before the programme, and right after the programme. Personal data collected in the questionnaires will be kept strictly confidential and will only be used for the purpose of research and analysis by SSPE/CUHK. Without your consent, no personal information will be released to any individual or organizations. This is the first questionnaires. Please complete it as soon as possible. Your full name is also required in the questionnaire for data matching. HKFA and SSPE/CUHK will follow up those who haven’t done the survey in the first class.

「賽馬會青少年暑期足球推廣計劃」是由香港賽馬會慈善信托基金撥款，並委託香港足球總會承擔發展，及香港中文大學體育運動科學系負責評估任務。現在，邀請您參與評估調查，協助了解足球發展計劃的情況。按撥款機構『香港賽馬會』的要求，作為足球計劃的重要部分，參加者務必填寫這份問卷調查。敬請按你自己認知，如實作答，你的回應並沒有對錯之分，祗要能反映你的真實情況便可。
